Dental Recordkeeping Courses
Professional, legal and ethical responsibilities require that dentists maintain complete records documenting all aspects of the patient’s dental care. The chart “tells the story”, beginning with the patient’s medical and dental history, continuing through diagnostic procedures and diagnosis, consent to treatment and treatment plan, and on to progress notes and referral documents. Charting is, quite simply, part of care.
These Dental Recordkeeping courses offer practical advice for understanding the dental recordkeeping requirements in Ontario and how to incorporate them into daily practice.
Fundamentals of Dental Recordkeeping
Course Description & Learning Objectives
Fundamentals of Dental Recordkeeping grounds participants in the critical principles and elements of patient records, increasing their knowledge and comprehension.
Upon successful completion of this course, participants will be able to:
- Explain the six components of a patient record that must be included to satisfy professional, legal and ethical expectations for documentation.
- Describe the content that must be included in each of the areas of the patient's record.
- Identify opportunities to strengthen their own practice and apply the fundamental principles of dental recordkeeping required to demonstrate they are practicing to the standard of care.
Mandatory Knowledge Assessment
Participants must successfully complete a multiple-choice knowledge assessment after attending the session.
Participants have two weeks after the session to begin their assessment(s), and once they begin, up to two hours to complete the 25 questions. A minimum score of 18/25 is required.
There is one opportunity to re-take the knowledge assessment for those who score lower than 18/25.
Certificates are only provided for those who achieve the minimum score. In other words, if a dentist attends the course but does not attempt or does not successfully complete the knowledge assessment, they are not entitled to any CE points.

Applied Dental Recordkeeping
Course Description & Learning Objectives
Applied Dental Recordkeeping provides an opportunity for participants to apply the principles to a range of case scenarios, solidifying learning and building confidence.
Upon successful completion of this course, participants will be able to:
- Apply the fundamental requirements of dental recordkeeping that demonstrate practice to the standard of care.
- Demonstrate the fundamental elements and content required to satisfy professional, legal and ethical expectations for documentation and explain why deficiencies pose a risk.
- Analyze their documentation and establish a plan to implement requirements of dental recordkeeping that demonstrate they are practicing to the standard of care.
- Evaluate a record to demonstrate proficiency in recordkeeping.
