As a result of its investigation of a formal complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. William Isen as follows: Dr. Isen must ensure that all of his patient records reflect the College’s Guidelines for Dental Recordkeeping. He must ensure that his clinical records contain detailed documentation of examinations, case work-ups, diagnosis, treatment planning, treatment, and so on, and he must ensure that what is billed clearly and directly corresponds with the treatment that is provided and that is documented. Even the perception that treatment and/or fees are unjustified, which can result from inadequate recordkeeping, can lead to loss of public confidence in the profession. Dr. Isen should be cognizant of his ethical obligation to uphold the public’s confidence in the profession. This issue is of particular concern when remediation has already been attempted with respect to other areas of clinical practice.
