As a result of a complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Keith Da Silva as follows: • Dr. Da Silva must understand the need to take radiographs at more regular intervals, particularly in respect of teeth that are more prone to developing caries at a faster rate, such as with hypoplastic teeth. • Dr. Da Silva must carefully review radiographs for the presence of caries and to follow up on and document areas that appear suspicious for caries. • Dr. Da Silva must document all patient visits to the office in the patient’s chart. • Dr. Da Silva must ensure accurate and complete documentation of patient appointments, including the taking of radiographs, radiographic findings and the performance of a recall examination with corresponding findings.
